Challenge

Smile America Partners is one of the largest school-based dental care providers in the United States, partnering with over 8,000 schools to deliver preventive services to children across 19 states. Their model is built around access: dental teams visit schools, set up mobile clinics, and provide preventive care to students who might not otherwise see a dentist.
Before a child can receive care, their parent must complete a consent form. Every form is handwritten, on paper, distributed through schools and returned via backpack mail. The logistics of school-based care create massive volume swings. Back-to-school season triggers a flood, then summer arrives and volume drops to nearly zero.
"Our form volume swings wildly week to week. Peak days meant overtime and weekends. Slow days meant staff sitting idle. We couldn't hire around that unpredictability," says Gary Bordey, CDIO of Smile America Partners.
The company had no automation, just staff typing every form by hand. At 300,000+ forms per year, the math simply didn't work.

Solution

The system needed to handle actual parent handwriting with all its variation. Cursive signatures. Block print. Spanish-language forms with accented names. Forms arrived wrinkled, folded, or torn from the journey through backpack mail.

DocuPipe's proof-of-concept validated it could handle the handwriting — including forms that had been impossible for traditional OCR. Smile America engaged DocuPipe's enterprise onboarding team to build their extraction schema, run accuracy analysis against real form data, and iterate until performance met production standards. The onboarding was hands-on and efficient: schema setup, performance tuning, and API integration were completed with minimal back-and-forth. Extraction accuracy paralleled what human data entry staff achieved — but at a fraction of the time and cost.

Smile America integrated DocuPipe into their backend via a custom API pipeline, completely replacing manual data entry.

Forms are scanned at local offices across 19 states. The moment a batch completes scanning at any location, the API ingests the documents, runs them through extraction, and routes data to Smile America's backend. Clinic staff scan forms and move on to patient care. Their job is healthcare, not document management.
Adding a new state or clinic doesn't require workflow changes. Smile America has expanded their geographic footprint multiple times since implementation, and each expansion required zero changes to document processing.
